Before Your First Visit

We currently have education modules available online and it is highly recommended that you begin reviewing these modules immediately. Much of the information from the modules can be applied and may have significant benefit to you as you wait for your appointment. Review of these modules will be mandatory in order to receive a New Patient Appointment. Information on how to access these modules online is located below.

 

We will contact you approximately 1 month prior to your New Patient Appointment booking. Please inform us at this time whether you have been able to complete these modules. If you cannot access, or do not have access to the mandatory education modules via computer, you can arrange to attend an in-house education session at this time. A bladder diary and patient history form will also be sent to you at time of booking for you to complete prior to your appointment. It is critical that you bring these with you to your first appointment.

 

Please ensure that your contact information is current. If updates are required, please have your referring doctor send our clinic this updated information as soon as possible.

 

Should there be a change in your symptoms or to the urgency of the referral, the referring doctor must send an updated referral request to our clinic.

Online Computer Education Modules Instructions

  1. Please visit the following website developed by our sister site in Calgary

  2. Select “Patient Education” from the menu on the home page.

  3. View modules 1 – 4 in the section marked Online Education Workshops.

  4. Once you are contacted about your New Patient Appointment, a bladder diary and patient history form will be sent to you to complete. You must bring this to your appointment.
